Daily benefit maximums by plan

GTL pays a daily benefit for each day you receive covered home health care services. Total benefits for all services are limited to the plan's combined daily maximum, over a maximum benefit period of 360 days (280 in Idaho).

To qualify, a licensed health care practitioner must certify a cognitive impairment or the inability to perform at least two of six Activities of Daily Living.

Prescription drug benefit

Your medications can pay for your policy

This is the benefit almost nobody expects. GTL reimburses $10 for each generic and $25 for each brand name prescription drug you fill, up to your plan's policy year maximum. For most people on several maintenance medications, that reimbursement offsets nearly the entire premium.

Family caregiver benefit

$3,500 in cash for the person taking care of you

Roughly 80% of home care is delivered by family. After you have an approved home health care claim and your caregiver registers and completes a caregiver assessment with TCARE, GTL pays a $3,500 lump sum. It can help cover out-of-pocket costs, travel and fuel, lost income from time off — or simply give them a chance to recharge. The money can be paid to you or assigned to your caregiver.

Additional benefits

Riders to customize your coverage

Accident & Sickness Hospitalization

$100 to $450 a day for hospital confinement, on a 3-, 6- or 10-day benefit period that restores after 60 days. Ages 75+: $100–$300 a day, 3- or 6-day period.

Ambulance Benefit

$200 per ground ambulance trip, up to four times a year and a $2,500 lifetime maximum. No hospital confinement required.

Dental & Vision

Up to $400, $800 or $1,200 a year after the first year, including $200 toward eyeglasses or contacts. Not available in GA, ID, MD or OR.

Critical Accident

$5,000 or $10,000 plan paying a lump sum after an ER visit for accidental death, fractures, dislocations and ligament tears. Not available in MD or WY.

Return of Premium

Returns all premiums paid, minus benefits paid, to your beneficiary at death. Not available in GA, MD, PA or TN.

Home Health Care Aide Benefit

$50, $100 or $150 a day when you cannot perform two Activities of Daily Living or have a cognitive impairment — no prior hospitalization needed.
